Admission Procedures
Persons interested in admission to a Personal Care Home should contact the continuing care nurse coordinator/social worker for their area.
An assessment will be completed by the nurse coordinator and a medical by their family physician.
The assessment will be reviewed by a Regional Committee of Health and Community Service who meet weekly. Approval is given when the level of care required by an individual is suitable for Personal Care Home, this being level one or two care.
Applicants and house owners will be advised of the discretion, however, admission is at the decision of the home owner/operator.
Outside Region Placements
Upon Request, assessment information will be forwarded to the appropriate placement coordinator outside this region.
There are a limited number of government subsidies available in some locations based on financial eligibility.
Persons must be assessed financially if required assistance with drugs and/or other services
